Abstract

Osteoarthritis (OA) is a progressive disorder that affects articular cartilage, subhondral bone and synovial membrane. Clinical and radiological findings are the mainstay of the diagnosis because a specific laboratory findings are lacking. Biomarkers are the fragments of proteins of the bone, cartilage and synovial membrane that can be detected in blood, urine and synovial fluid. Many protein products of the joint metabolism have been recognized but its role in disease process remains unclear. To optimize results it is important to determine time of a day, food consumption and patient activity due to sampling, to standardize analytic procedures and refferal values and to validate results. Diverse results of many investigations hamper the final agreement regarding the usage of particular biomarker in OA. It seems that urinary carboxil-terminal cross-linked telopeptide of type II collagen (uCTX-II), hyaluronic acid (HA) and cartilage oligomeric matrix protein (COMP) are the most promising biochemical indices in OA regarding disease activity.
