Abstract

Chronic wounds can be either of venous or
arterial pathophysiological origin, and they represent an actual
dermatological problem. Chronic venous insuffi ciency causes
crural cutaneous changes due to the pathological venous circulation
(destruction of vein valves or chronic venous obstruction).
Venous leg ulcer occurs in the distal part of the shin, mostly
around the medial malleolus. The base of the ulcer is often
covered by yellowish fi brin layer. The surrounding skin is hyperpigmented,
usually irritated by ulcer secretions and infl amed.
Arterial leg ulcer is caused by arterial insuffi ciency. It is round
in shape, with dry base, and involves deeper structures, up to
the tendons. The skin is dry, atrophic, cold, pale, smooth, and
hairless. Ulcer therapy is demanding, and consists of numerous
general measures, and topical and/or systemic therapy.
