VACUUM – ASSISTED DELIVERY

Abstract
Objective. To analyse the effect of vacuum-assisted delivery (VE) in the completion of delivery on ¬morbidity and mortality of both newborns and women and to discuss the controversy of this method. Methods. The ¬outcomes of the spontaneous and VE deliveries in Virovitica County Hospital were analysed in the period 2002.–2006. Results. In the time investigated 113 (2,59%) deliveries were VE completed. In the group of 99 VE (for the omitted 14 was ¬incomplete information) in relation to 295 spontaneous vaginal deliveries there is a significantly more frequent occurence of primiparas (66,7% vs. 47,1%, p=0,006), a longer duration of delivery (7,58±3,67 vs. 6,22±3,37 hours, p=0,001), more frequent lesion of soft tissues (23,3% vs. 5,08%), a signifficantly smaller value of 1-st. minute Apgar score (7,82±1,68 vs. 9,33±1,57, p=0,0001) and 5-th minute Apgar score (9,28±1,09 vs. 9,77±0,74, p=0,0001). VE indications were: lack of ¬advancement with a prolonged duration of the second birth period, fetal asphyxia and the condition after the caesarean section. During the same period 1541 (37,94%) newborns were hospitalized at the Department of Neonatology, 23 (23,2%) of them were from the VE group. Conclusion. The results indicate that VE has an effect on mothers' and ¬newborns' morbidity, on the number of slight perineal tears and mild complications of newborns, but does not have an ¬effect on the increase of perinatal mortality and long-term treatment of the newborns. VE method should be used, with a good judgement, proper indication, skill and technique, and with regard to possible complications.
