Abstract

Streptococcus pyogenes causes a wide range of human diseases, from sore throat and impetigo to severe invasive infections. Species of macrolide-resistant S. pyogenes have increasingly been encountered all over the world and it has been suggested they possess a more invasive potential. The infections caused by S. pyogenes represent 41 % of all reported infectious diseases in the Primorsko-Goranska County in the period between 2001 and 2006. Annually there were 297 to 511 cases of streptococcal pharyngitis, 132 to 178 cases of erysipelas and 222 to 404 cases of scarlatina. S. pyogenes resistance to macrolides was detected in the range of 10 to 27 % and to clindamycin from 3 to 21 % respectively. In the same period, the resistance to macrolides in Croatia ranged from 11 to 16 % and to clindamycin from 3 to 9 %. Increasing S. pyogenes resistance to macrolides poses a need for review of clinical guidelines for use of antimicrobial drugs in the empirical treatment of infectious diseases in certain areas of the Republic of Croatia.
