Platelet Aggregation Disorder Following DTP-Polio-HIBERIX Immunization

Abstract

Most commonly described adverse reactions occurring 24 hours after immunization are local reactions and fever. Systemic reactions have been described very rarely such as seizures, hypotonic-hyporesponsive disorders, gastrointestinal disorders, agitation, nausea, and prolonged crying and screaming. Acute allergic reactions have also been reported but very rarely: dyspnea, cyanosis, angioneurotic edema, hypotension, and very rarely: anaphylactic shock. A reaction of local edema and cyanosis, redness, purpura and extreme crying have also been reported. A5-month-old child was admitted to Pediatric Ward of Požega County Hospital for the first time. The boy's mother noticed a purpura rash the day after DTP-polio + Hib immunization. He had a latent infection which was treated with antibiotics, and a complete aggregation disorder of the platelets. A possible adverse reaction to immunization was suspected and the child was treated with fresh frozen plasma and corticosteroids. The patient gradually recovered (purpura disappeared, and platelet function recovered gradually). During the following two years the patient was not immunized. An agreement had been made with the child's parents that the boy would be admitted to hospital were he was to be vaccinated. On admission the platelet function (aggregation) was adequate. Then the child was vaccinated (Pediacel vaccine), and observed. Platelet function changed: a disorder in aggregation was obvious 24 hours after the administration of DTP-polio HIB vaccine, this time without clinical symptoms. During the next 24 hours a spontaneous gradual recovery of the platelet function was evident.
