Hyperbaric oxygenation in the treatment of central nervous system infections

Hyperbaric oxygenation treatment (HBOT) is a method of treatment that provides patients in specially designed chambers to respire 100% oxygen at pressures above 100 kPa (1,0 bar). With every pressure increase of 100 kPa, additional 2,4 ml of oxygen are physically dissolved in 100 ml of blood and delivered to the tissues. The method is used in the treatment of diseases that are, directly or indirectly, accompanied by hypoxia. Although the efficiency of HBOT in the treatment of many diseases is still a matter of concern and suspicion, clinically tested effects of this method make it the treatment of choice in some diseases, while in the treatment of other diseases it represents an important remedy or an important adjunct. HBOT is definitely not "a remedy for all diseases" but merely a powerful weapon used to correct hypoxia. In clinical practice, of outmost importance are two types of infections of the central nervous system (CNS), postoperative infections and bran abscess. Brain abscess requires quick, efficient and dedicated therapy, but the best therapeutic protocol has not yet been uniformly accepted. The rationale for the use of HBOT in the treatment of brain abscess is based on its bactericidal power, synergistic action with some antibiotics, as well as on the ability to lessen brain edema and lower intracranial pressure. The usage of HBOT in the treatment of CNS infections enables shorter hospital stay, less reoperations, shorter duration of antibiotic treatment and lower overall treatment costs. HBOT is provided at the pressures usually ranging from 2,2 to 2,5 bars, during 60 to 90 minutes, once or twice per day. However, it still remains unclear if it is possible to define criteria for the usage of HBOT as a part of therapeutic protocol of CNS infections and if its usage is ethically acceptable, medically safe and financially reasonable.
