Tuberculosis of the peritoneum - Case report

Sažetak

Background. Our paper presents an extra-peritoneal form (seat) of tuberculosis caused by Mycobacterium tuberculosis which grew in the culture of sputum and ascites, with histological findings of granulomatous inflammation of the peritoneum in patients with previously undiagnosed primary pulmonary tuberculosis.
Methods. Analysis of epidemiological and clinical data from medical history, laboratory analysis of blood parameters, radiological, histological and microbiological studies in the diagnosis of tuberculosis.
Case report. A previously healthy 25-year-old man was hospitalized due to fever, abdominal pain and ascites. X-ray examination of the lung showed an inhomogeneous infiltrate with transparency (18 mm) in the apical region of the right lung. CT of the thorax region showed an infiltration with decomposing cavity, the CT of the abdomen and pelvis showed an extensive ascites, and MRI of the abdomen showed an subcapsular collection (thickness 32 mm) in the right lobe of the liver. Microbiological examination of the sputum and ascites culture showed growth of Mycobacterium tuberculosis. The histological analysis of parietal peritoneum tissue, taken by laparoscopic method, showed granulomatous inflammation. Effective treatment was achieved by using anti tuberculosis drugs such as isoniazid (INH), rifampicin (RIF), pyrazinamide (PZA), streptomycin (SM).Discussion and conclusion. The established methods are not sufficiently suitable and efficient in the diagnosis of extrapulmonary tuberculosis, and tuberculosis of the peritoneum, and it is recommended to use the laparoscopic method to obtain clinical specimens and molecular techniques for pathogen detection.
