Prevalence of endometriosis in four regions of Croatia based on data from the Central Health Information System

Sažetak
Background: Epidemiological data on endometriosis is of importance for public health decisions and funding. The reported prevalence rates of endometriosis are, however, elusive, due to various methods for confirmation of diagnosis used as well as due to different populations recruited in diverse studies.
Objectives: The epidemiological data on endometriosis in Croatia are scarse. The aim of this study was therefore to assess the time trends and age and regional distribution of the endometriosis prevalence in Croatia by analyzing the N80 ICD-10 groups of codes extracted from health insurance data for the period 2015 – 2023.
Materials and Methods: The study analyzed administrative data from The Croatian Central Information System, which collects healthcare related data for all Croatian residents. The collected data included between 766 969 women in the reproductive age (15 – 49 years) in 2023 and 904 589 women in 2015.
Main outcome measures: Time trends, regional trends and age-related distribution of point prevalence rates in the 9 - year period were analyzed using covariance analysis.
Results: The prevalence rate of endometriosis diagnoses showed constant increase over the analyzed 9-year period, rising from 3.7/1,000 women in 2015 to 6.1 in 2023, except for the year 2020. The peak prevalence rates were in the 35 to 44 years age group. The city of Zagreb had constantly the highest prevalence rate which has been increasing from 5.8/1,000 women in 2015 to 8.7/1,000 women in 2023. The regions of Adriatic Croatia and of Pannonian Croatia had constantly lower prevalence rates when compared to other two NUTS regions.
Conclusions: The endometriosis point prevalence rates in Croatia, calculated through the analysis of health insurance data are in concordance with similar investigations of health insurance data in other countries that show the constant increase of prevalence with each passing year. The NUTS regions of Adriatic and Pannonian Croatia had lower prevalence rates than City of Zagreb and the North Croatia region.
