Sažetak
The role of the nurse during a mass transfusion (MT) is multiple and at the same time poorly defined. The nurse contributes to the treatment of bleeding patients depending on the level of education and competence and the role within the team.
Before the MT begins and the protocol is activated, it is the nurse/medical technician who notices the first signs that the patient is bleeding. Through modern monitoring methods and guided by experience, it is they who are next to the patient when it all starts and who are the first to start what soon becomes MT.
In the process of meeting the needs of bleeding patients, many challenges arise that the nurse must autonomously address, while at the same time participating in the work of the team. Organizational challenges are one of the biggest factors that impose themselves, especially when every second counts. They appear on all sides, from the coordination of the team of nurses/medical technicians, to the coordination of that same team within a larger multidisciplinary team in the intensive care unit (ICU), and finally to the coordination with services within the hospital, such as the laboratory and transfusion. The Republic of Croatia (RC) does not have protocols for MT, and thus does not have protocols for the nursing organization within MT, which is not surprising when such protocolized guidelines have only just begun to appear in the world's largest medical centers.
Nurses/medical technicians who fulfill organizational roles in the MT process rely on their skills, experience, and knowledge. Which is a lot in itself, but surely the existence of easily applicable protocols could provide satisfactory results in a coherent relationship with that same knowledge and experience.
