Reconstructive Surgery in the Treatment of Scalp Squamous Cell Carcinoma: A Case Report

Sažetak
Aim: This case report highlights the importance of an interdisciplinary approach to the treatment of squamous cell carcinoma (SCC), where collaboration between dermatology and reconstructive surgery ensures an optimal therapeutic outcome for the patient. Case report: An 89-year-old male was referred to a plastic surgeon due to a pronounced defect following resection of a squamous cell carcinoma located on the scalp, frontoparietally on the left. Dermatologists performed a radical resection, after which the disease was determined to be locally advanced but without evidence of regional or distant metastases. The defect was reconstructed with a local occipital rotation flap. The secondary defect in the occipital region was covered with a split-thickness skin graft harvested from the right thigh and secured with a VAC device. The rationale for combining these techniques was the periosteal damage at the tumor site; the use of a split-thickness skin graft alone would not have been successful due to poor vascularization, leading to graft failure. The postoperative course was uneventful, with highly satisfactory functional and aesthetic results, and without signs of complications or disease recurrence. Conclusion: The incidence of squamous cell carcinoma is rising, with studies showing it has doubled over the past three decades, from nearly 14 to 23 newly diagnosed cases per 100,000 inhabitants. The increase is particularly evident among the elderly and individuals with chronic UV exposure. Although the cure rate is high, early diagnosis and appropriate therapeutic management are essential, with surgical treatment remaining the gold standard. Successful treatment requires an interdisciplinary approach. In cases such as this, accurate dermatological diagnosis and proper reconstructive surgery are crucial to preserving both function and aesthetics of the affected area, using techniques tailored to the size and location of the defect.
