Acute rehabilitation of spinal cord injury

Abstract

Traumatic spinal cord injury presents with loss of function of neuromuscular and other systems below the level of injury. Patients may suffer from minor loss of strength to complete quadriplegia with respiratory distress. All the patients with traumatic spinal cord injury who are admitted and treated in University Medical Centre Ljubljana are evaluated after admission and individualized plan of rehabilitation is made.
The neurological level of injury is documented with international standards for neurological classification of spinal cord injury proposed by ISCOS-ASIA.Manual muscle testing of all the mucsles is done. Scales that are used to evaluate the outcome are DeMMI scale, SCIM, extended Barthel index and VAS.
The individualized rehabilitation program takes into account the limitations of surgical and intensive care treatment. Physiotherapy interventions for spinal cord injury patients consist of training of spare muscle groups, bed cycling (passive or active), NMES of muscles one or two levels below the motor neurological level, verticalisation on tilt table, sitting and in cases of paresis also walking. In the occupational therapy interventions the activity of daily life and the transfer to the wheelchair are trained and the individual orthosis are made. The swallowing and speaking are assessed and the respiratory physiotherapy is included when needed. The SCI patients also get psychological support. The special care for sphincter control, deep vein thrombosis and prevention of nosocomial infections is incorporated in the rehabilitation plan.
All SCI patients are refered to further rehabilitation according to their neurological level and assessed impairment.
