ASPERGILLUS SINUSITIS IN A RENAL TRANSPLANT RECIPIENT: A CASE REPORT

Abstract

Invasive fungal sinusitis is rare in renal transplant population. We present a case of 65-year-old male patient who developed slowly progressive rhinosinusitis with tissues destruction. Amphotericine B resistant Aspergillus fl avus and voriconasole sensitive Aspergillus sclerotiorum were isolated as causative pathogens. Voriconasole therapy was initiated along with reduction of the immunosuppression. Frequent disease relapses required prolonged voriconasole treatment, which was later switched to caspofungine because of development of resistance. At the last outpatient control, he was stable, without infective symptoms, with stable renal allograft function. In conclusion, paranasal sinuses aspergillosis is rare in renal transplant recipients. Early diagnosis with aggressive surgical treatment and resection of necrotic tissue, accompanied with the long term antifungal therapy increases the probability of favourable outcome.
