Abstract
Individuals with severe mental disorders comprising schizophrenia, major depressive disorder and bipolar affective disorder have a life expectancy 15 – 25 years shorter compared to individuals from the general population. The high rate of unnatural deaths (predominantly, suicides) significantly contributes to reduced life expectancy among individuals with severe mental disorders. Like among individuals from the general population, cardiovascular diseases have been recognized as a main contributing factor to reduced life expectancy among individuals with severe mental disorders. Many cardiovascular risk factors among individuals with severe mental disorders, such as unhealthy dietary pattern, cigarette smoking and reduced physical activity are also present among individuals from the general population. Additional risk factor among individuals with severe mental disorders include the use of psychotropic medications, shared genetic vulnerability and intriguing interaction between cardiovascular risk factors. In the research literature, there is still a lack of studies on systematic assessment of cardiovascular risk factors among individuals with severe mental disorders, specifically on their interaction. In clinical practice, comorbidity of cardiovascular diseases and severe mental disorders is likely to be underrecognized and undertreated. In this article we discuss the risk factors for cardiovascular diseases among individuals with severe mental disorders, highlighting the role of multidisciplinary approach comprising psychiatry and cardiology teams in prevention strategies. Furthermore, we provide novel insights related to the genetic relationship between severe mental disorders and cardiovascular diseases, based on genome-wide association studies. Finally, we discuss clinically significant interactions between common cardiovascular and psychotropic medications and neuropsychiatric effects of common cardiovascular medications.
