Treatment of Patients with Medication-overuse Headache

Medication-overuse headache (MOH) is a type of chronic secondary headache caused by regular and excessive use of medications for acute symptomatic treatment of primary headache. Although the prevalence of MOH in the general population does not exceed 1.7%, it has a negative impact on the quality of life, thus creating an economic burden for the entire society. The International Classification of Headache Disorders, 3rd edition (ICHD-3) defines MOH and classifies it into 8 subtypes depending on the type of overused medications. Primary headaches most commonly associated with MOH are chronic migraine and tension-type headache. MOH comorbidities primarily include affective disorders, which requires interdisciplinary treatment approach. The treatment of MOH, which is often recurring, requires withdrawal from the acute medication causing it, i.e. detoxification. Detoxification can either mean gradual reduction, or complete withdrawal from acute medication, and can be carried out through outpatient or inpatient programs. MOH prophylactic treatment is based on the prophylactics of the underlying primary headache. The development of medication-overuse headache can be prevented by timely identifying the chronification of primary headache and by starting prophylactic treatment.
