Venous Thromboembolism Prophylaxis in Major Orthopedic Surgeries

Sažetak

Venous thromboembolism (VTE) is a well-known complication after major orthopedic procedures such as total hip and knee arthroplasties and following hip fractures. Existing clinical guidelines for the use of thromboprophylaxis greatly facilitate daily clinical work, and have one main goal: how to balance the risk of VTE with the risk of perioperative bleeding. However, most of these guidelines are outdated and do not take into account advances in the application of innovative perioperative protocols. Recent data suggest that acetylsalicylic acid may be appropriate for most patients after total hip and knee arthroplasty, and direct oral anticoagulants (DOACs) are preferred in high-risk patients. Low molecular weight heparin (LMWH) remains the recommended choice after hip fracture surgery, but also a very good and safe choice for prophylaxis following the hip and knee arthroplasty. The aim of this review is to present the latest knowledge and guidelines for the use of thromboprophylaxis after major orthopedic operations.
